(1) An application is
to —
(a) be
in writing;
(b) be
made in an approved manner and form;
(c)
specify the area, or areas, of medical radiation technology in which the
applicant seeks registration; and
(d) be
accompanied by the application fee, if any, prescribed by the regulations.
(2) The applicant must
provide the Board or the registrar with such further information as the Board
or the registrar requires, in any particular case, and if required by the
Board or the registrar must verify the information by statutory declaration.
(3) The Board may, in
writing, require an applicant for registration to attend before the Board for
the purpose of satisfying the Board as to a matter relevant to the application
and, if the person fails to attend, may refuse the application.
(4) The Board may
refuse to register a person who does not comply with this section.
