(1) If a person has
lost their statutory certificate, or the person’s statutory certificate
is destroyed, the person may apply to the regulator for a replacement
certificate.
(2) The application
must be —
(a) in
writing; and
(b)
accompanied by a statutory declaration stating that the person has lost their
statutory certificate or it has been destroyed.
(3) If the regulator
is satisfied that the person has lost their statutory certificate, or it is
destroyed, the regulator may issue a replacement certificate.
